(<\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/28716514\/\"><span style=\"font-weight: 400;\">Morozova et al., 2017<\/span><\/a><span style=\"font-weight: 400;\">). If toe walking persists beyond age three, it may be an early symptom of an underlying developmental condition. Persistent toe walking may also indicate short or tight Achilles tendons, cerebral palsy, or, in rare cases, muscular dystrophy (<\/span><a href=\"https:\/\/journals.lww.com\/jbjsjournal\/Abstract\/2018\/04180\/Idiopathic_Toe_Walking__Prevalence_and_Natural.2.aspx\"><span style=\"font-weight: 400;\">Engstr\u00f6m and Tedroff, 2018<\/span><\/a><span style=\"font-weight: 400;\">; <\/span><a href=\"https:\/\/online.boneandjoint.org.uk\/doi\/full\/10.1302\/1863-2548.13.180160\"><span style=\"font-weight: 400;\">Leyden et al., 2019<\/span><\/a><span style=\"font-weight: 400;\">). Studies on the prevalence of idiopathic toe walking did not differentiate between ASD and other neuropsychiatric conditions until recently. However, in 2019 a large-scale review of trends and treatment patterns found that <\/span><a href=\"https:\/\/online.boneandjoint.org.uk\/doi\/pdf\/10.1302\/1863-2548.13.180160\"><span style=\"font-weight: 400;\">9% of children with ASD<\/span><\/a><span style=\"font-weight: 400;\"> are diagnosed with persistent\/idiopathic toe walking compared to less than 0.5% of children without an autism diagnosis (Leyden et al., 2019).\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Causes for this increased prevalence of toe walking in individuals on the spectrum are varied and unclear. In some cases, it results from tight heel cords that restrict the range of movement in the ankle (Barrow et al., 2011). Toe walking can also be caused by a dysregulated <\/span><a href=\"https:\/\/autism.org\/sensory-integration\/\"><span style=\"font-weight: 400;\">vestibular<\/span><\/a><span style=\"font-weight: 400;\"> or sensory system involving touch, proprioception, and vision. More research is needed to fully understand which tactile\/sensory processing differences may cause idiopathic toe walking in children with ASD.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The long-term effects of toe walking are not fully understood. Some studies affirm a connection between toe walking and <\/span><a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/abs\/10.1002\/aur.2009\"><span style=\"font-weight: 400;\">secondary shortened Achilles tendons<\/span><\/a><span style=\"font-weight: 400;\"> or other <\/span><a href=\"https:\/\/journals.lww.com\/pedorthopaedics\/Abstract\/2006\/09000\/Comprehensive_Outcomes_of_Surgically_Treated.9.aspx\"><span style=\"font-weight: 400;\">biomechanical changes<\/span><\/a><span style=\"font-weight: 400;\"> which can limit the range of motion in the feet and ankles (Valagussa et al., 2018; Mulkin et al., 2006). Others assert that issues with toe walking are <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3565400\/\"><span style=\"font-weight: 400;\">purely cosmetic<\/span><\/a><span style=\"font-weight: 400;\"> and have no long-term adverse physical effects (Dietz and Khynsree, 2012).\u00a0<\/span><\/p>\n<h2><strong>Available Treatments<\/strong><\/h2>\n<p><span style=\"font-weight: 400;\">There are several treatments available that can reduce or eliminate toe walking. Before starting a therapeutic regimen, we advise that you seek information on available treatments and their long-term effectiveness. Some interventions <\/span><a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/abs\/10.1111\/cch.12745\"><span style=\"font-weight: 400;\">lack clinician support and significant evidence<\/span><\/a><span style=\"font-weight: 400;\"> of efficacy and <\/span><a href=\"https:\/\/www.cochranelibrary.com\/cdsr\/doi\/10.1002\/14651858.CD012363.pub2\/full\"><span style=\"font-weight: 400;\">no single treatment<\/span><\/a><span style=\"font-weight: 400;\"> has demonstrated extensive long-term success (Williams et al., 2020; Caserta et al., 2019).<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Treatments for toe walking in a child with autism include:\u00a0<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Physical therapy<\/b><span style=\"font-weight: 400;\"> &#8211; A physical therapist works with the child on stretching the <\/span><a href=\"https:\/\/journals.lww.com\/pedpt\/fulltext\/2019\/10000\/effect_of_plantar_flexor_muscle_strengthening_on.18.aspx\"><span style=\"font-weight: 400;\">leg and foot muscles<\/span><\/a><span style=\"font-weight: 400;\">; this can help release tension and increase range of motion (de Oliveira et al., 2019).<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Prism Lenses<\/b><span style=\"font-weight: 400;\"> &#8211; An optometrist with expertise in vision training may prescribe ambient prism lenses. These lenses help improve one&#8217;s perception of where one&#8217;s body is relative to their surroundings.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/autism.org\/sensory-integration\/\"><b>Vestibular Stimulation and Sensory Integration<\/b><\/a><span style=\"font-weight: 400;\"> &#8211; A physical or sensory therapist may recommend that your child swing, spin, or engage in other actions that stimulate the vestibular system.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Leg braces or splints<\/b><span style=\"font-weight: 400;\"> &#8211; A physician may prescribe a leg brace or splint to help promote contact between the heel and the ground.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Serial Casting <\/b><span style=\"font-weight: 400;\">&#8211; A physician may apply a series of below-the-knee casts to <\/span><a href=\"https:\/\/regispthomeexerciseprograms.pbworks.com\/f\/Serial+casting+in+the+treatment+of+idiopathic+toe+walking+and+a+review+of+the+literature.pdf\"><span style=\"font-weight: 400;\">progressively adjust<\/span><\/a><span style=\"font-weight: 400;\"> the foot&#8217;s position (Fox et al., 2006).\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/europepmc.org\/article\/med\/10647110\"><b>Surgery<\/b><\/a> <span style=\"font-weight: 400;\">&#8211; A surgeon may lengthen the muscles or tendons in the back of the lower leg. This option is usually only recommended when other therapies have failed (Eastwood et al., 2000).\u00a0<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">As stated, toe walking is generally not a cause for concern in children under three years of age. If your child is older and regularly toe walks, consider consulting your child&#8217;s pediatrician. They may perform an assessment or refer your child to a specialist to determine which course of action (if any) to take. It is essential to work closely with your care team when deciding if\/how to treat your child&#8217;s toe walking.<\/span><\/p>\n<\/div><div class=\"fusion-separator fusion-full-width-sep\" style=\"align-self: center;margin-left: auto;margin-right: auto;margin-top:30px;width:100%;\"><\/div><\/div><\/div><\/div><\/div><div class=\"fusion-fullwidth fullwidth-box fusion-builder-row-2 fusion-flex-container has-pattern-background has-mask-background nonhundred-percent-fullwidth non-hundred-percent-height-scrolling\"
